Once you have registered online, we will need these items to complete enrollment: 

Current immunizations

Certified birth certificate

Contact information for the previous school (e.g., name, address, phone, fax)

Proof of residency (Accepted: lease/buyers agreement, current water or electric bill)

Physical (if entering a Kansas school for the first time and between the ages of 5-9) 

Transfer Form (Due by March 31st)

Not returning in the fall?

Please let us know and have your new school send all records requests to rthompso@usd497.org so we can forward your student's records.
