ATHLETIC FORMS INFORMATION:

Every athlete is required to submit five (5) athletic forms (#1-5 below) in their entirety to the BMMS main office prior to participation in any sports practice or tryouts regardless of whether tryouts are held. 

Every manager is required to submit three (3) athletic forms (#2-4 below) in their entirety to the BMMS main office prior to participation with any sports teams practice or tryouts. 

Do not submit forms to coaches at tryouts/practices. Athletic packets must be turned in to the main office. 

REQUIRED FORMS:

1. KSHSAA-PPE - Physical Form 

• Requires the signature of the doctor on page 2. Students and parents must sign the front and back of the form.

• The annual physical examination shall not be taken earlier than May 1 preceding the school year for which it is applicable. 

2. LPS Insurance Compliance Form

• Requires parent signature and notarization - Form must be signed in front of a notary, public.

• A notary may be available in the front office - please call ahead. 

• Every athlete and manager is required to have accident insurance. If you are not currently covered by health insurance, you may purchase accident insurance through Student Assurance Services. Forms are available at BMMS.

3. KSHSAA Concussion Release Form 

• Requires the signature of the student and parents

4. LPS Code of Conduct Form 

• Requires the signature of the student and parents

5. LPS Tryout/ Participation Form

• Requires the signature of the student and parents
