Physicals and Athletics Participation

A physical form must be on file for all athletes participating on a Lawrence Public School athletic team. A Kansas State High School Activities Association (KSHSAA) Pre-Participation Physical Evaluation Form (PPE) must be signed and dated by a licensed health provider. Health providers must be one of the following: MD, DO, DC, PA-C, or APRN. Physicals for current school year participation may not be dated before May 1 of the year preceding participation. Physicals dated before May 1 will not be considered valid.  

Along with the PPE, an athlete is required to submit proof of insurance coverage. A Lawrence Public Schools Athletic Insurance Compliance form is required for all athletes and student managers. Insurance Compliance Forms dated before May 1 of the year preceding participation.
