    IMPORTANT: ASQ Questionnaire!

    Because your child's first 5 years of life are so important, we want to help you provide the best start for your child.  You are asked to participate in the completion of the Ages & Stages Questionnaires®: Social Emotional, Second Edition (ASQ:SE-2™) and Ages & Stages Questionnaires® (ASQ-3), online screenings which provide a quick check of your child’s overall development. The questionnaires include questions about your child's communication, gross motor, fine motor, problem solving, personal social skills, and behavior.
