• REQUIRED FORMS FOR ATHLETIC PARTICIPATION
    Every Athlete or Student Manager is required to submit athletic form for participation:
    Athletes: five (5) athletic forms (#1- #5 below)
    Managers: student manager application and three (3) athletic forms (#2-#4 below)
     
    All forms must be completed in their entirety and submitted to the LHS Athletic Office prior to participation in any sports practice or tryouts regardless of whether tryouts are held
     
    • Deadlines for submitting forms will be posted and will run in the upcoming important date/deadlines section of this website
    • Forms submitted after the deadline may not be processed in time for participation in tryouts or practices on the first day
    • Incomplete forms will not be accepted and will be returned for completion
    • Forms may NOT be submitted to coaches at tryouts/practices or to summer weights staff
     
    REQUIRED FORMS:
     
        1) KSHSAA-PPE - Physical Form
    Requires the signature of doctor on page 2. Student and parent must sign the front and back of form
    • The annual physical examination shall not be taken earlier than May 1 preceding the school year for which it is applicable
    • Doctor's signature MUST be signed and dated by an MD, DO, DC, PA-C or APRN ~ KSHSAA
    • If the forms is not completed in it's entirety it will be returned and the athlete may not participate until completed
       2) LPS - Insurance Compliance Form
    Requires parent signature and notarization ~ Form must be signed in front of the notary
     
    NOTE: Every Student-Athlete and Student-Manager is required to have accident insurance. If you are not currently covered up to $25,000 by health insurance you may purchase accident insurance through Student Assurance Services. The enrollment and claim forms are available at the school. Coverage begins the day after payment is received and ends the last day of school. 
     
     
     3) KSHSAA - Concussion Release Form
    • Requires the signature of the student and parent
     
       4) LPS - Code of Conduct Form
    • Requires the signature of the student and parent
    • Updated August 2019 - All athletes are held accountable to the current policy regardless of when a signed form submitted 
      
       5) LPS - Tryout/Participation Form
    • Requires the signature of the student and parent
     ***The KSHSAA has updated the PPE form. Prior versions will not be accepted
     
     
    OPTIONAL FORMS 
     
    LAWRENCE HIGH SCHOOL - Transcript Release Form (RECOMMENDED)
    Suggested for Junior and Senior Students who may pursing post-secondary academic, athletic, or military careers 
    Transcript Requests by Post-Secondary academic, military, and vocational institutions
     
    Post-secondary academic, military, and vocational institutions may request copies of transcripts for students and/or student-athletes they are considering admitting to their institution. A signed transcript release form must be on file and recorded for the LHS Registrar or Athletic Department to fulfill the request(s).
    • Requires the signature of student and a parent/legal guardian for all students under 18 years of age 
     
    LAWRENCE HIGH SCHOOL - Parent Request to Transport Own Student (NOT A BLANKET FORM - MUST BE SUBMITTED FOR EACH DATE PARENT TRANSPORT IS REQUESTED)
    REQUIRED if a parent/legal guardian would like to transport their own student to/from a school sponsored activity or athletic event. It is the expectation that all student-athletes travel to and from contests with their team. Parents/legal guardian my request an exception in the event of a family hardship or emergency.
    • Signed form must be submitted to the Activity Director or Athletic Director a minimum of 24 hours in advance 
    • Requires Coach/Sponsor/Teacher signature prior to submission to administration for approval
     
    STUDENT ASSURANCE OPTIONAL ACCIDENT INSURANCE
    As a service to the community, Lawrence Public Schools has contracted with Student Assurance Services to provide optional student accident insurance. This is a valuable option to parents/guardians who, because of various reasons, do not have health insurance. It is also an excellent way to supplement health insurance if you have a large deductible or co-insurance, including HSA plans.
